Summary

Focus on Ensuring quality care, patient satisfaction, and cost- effectiveness throughout a patient‘s Stay. Acting as Patient advocates, coordinators, and communicators through a patient’s journey.
  
Principle AccountabilitiesAssess patient’s needs and develop individualized care plans in collaboration with doctors, nurses and other healthcare Professionals.
Duties & Responsibilities1Ensuring a smooth transition from hospital to next care setting, whether it’s home care, rehabilitation, or assisted living.
2Develop a Comprehensive Care Plan outlining treatment options and post-discharge needs.
3Assess a Patient’s medical, social and Psychological needs to understand their overall situation.
4Educate Patients and their families about their condition, treatment options and the overall care plan.
5Empower Patients to make informed decisions about their healthcare.
6Act as a liaison between patients, families and the healthcare team, ensuring clear communication and addressing any concerns.
7Connects Patients with social workers to address social determinates of health, like housing or financial assistance.
8Contribute to quality assurance initiatives within the hospital, ensuring adherence to best practices and patient safety protocols.
9Monitor Patient’s Progress, adjust the care plan as needed and document everything for continuity of care.
10Analyze the appropriateness of a patient’s treatment plan, ensuring it’s medically necessary and delivered in the most cost-effective setting.

Educational RequirementsBachelor of Medicine Experience Requirements2 Years’ experienceKnowledge, Skills and AttitudeMust have integrity, initiative, evidence of good health and grooming.Participates actively in organization-wide  activities without hesitance or personal reservation.   Can adapt to unusual job-related circumstances. Works effectively under stressful condition.  .
