Principle Accountabilities | Assess patient’s needs and develop individualized care plans in collaboration with doctors, nurses and other healthcare Professionals. |
Duties & Responsibilities | 1 | Ensuring a smooth transition from hospital to next care setting, whether it’s home care, rehabilitation, or assisted living. |
2 | Develop a Comprehensive Care Plan outlining treatment options and post-discharge needs. |
3 | Assess a Patient’s medical, social and Psychological needs to understand their overall situation. |
4 | Educate Patients and their families about their condition, treatment options and the overall care plan. |
5 | Empower Patients to make informed decisions about their healthcare. |
6 | Act as a liaison between patients, families and the healthcare team, ensuring clear communication and addressing any concerns. |
7 | Connects Patients with social workers to address social determinates of health, like housing or financial assistance. |
8 | Contribute to quality assurance initiatives within the hospital, ensuring adherence to best practices and patient safety protocols. |
9 | Monitor Patient’s Progress, adjust the care plan as needed and document everything for continuity of care. |
10 | Analyze the appropriateness of a patient’s treatment plan, ensuring it’s medically necessary and delivered in the most cost-effective setting. |
