We are seeking data-driven medical doctors to leverage EMR data to proactively identify care gaps and design personalised care plans for US nursing home residents. This is a non-traditional, highly analytical clinical role focused on population health outcomes rather than direct patient care.
Responsibilities Analyze large-scale EMR and healthcare data to identify care gaps, clinical risks, and opportunities for intervention among nursing home residents. Review resident populations to detect unmet preventive, chronic disease, medication, and follow-up care needs. Develop personalized, evidence-based care plans tailored to individual residents' medical histories, risk factors, and health goals. Apply clinical judgment to translate data insights into actionable recommendations for care teams. Stratify residents by risk and prioritize interventions for high-risk or medically complex populations. Monitor population health metrics and evaluate the effectiveness of care plans and interventions. Collaborate with nurses, care coordinators, pharmacists, and other healthcare professionals to optimize resident outcomes. Use clinical guidelines and best practices to support quality improvement initiatives. Identify trends in hospitalization, readmission, medication use, chronic disease management, and preventive care performance. Support value-based care, quality measurement, and population health programs through data-driven recommendations. Participate in the design and refinement of clinical algorithms, risk models, and decision-support tools. Ensure clinical recommendations comply with regulatory requirements and quality standards relevant to U. S. nursing homes. Document clinical assessments, rationale, and care recommendations accurately within internal systems. Contribute to continuous improvement of workflows, care pathways, and outcome measurement processes.
Qualifications A bachelor's degree in medicine is a must. A proven previous experience in a relevant full-time job is a must. Excellent verbal and written communication skills. B2 English level is a must.
