- Attend the nurses' endorsement between night and day shifts, getting an overview of their new cases and an update about their old cases.
- Proceeding to early discharge management and facilitation for patients who have fulfilled their care
- review labs, reds, procedures, and medical consultations requested by their patients, making necessary communications with relevant personnel to improve the quality and promptness of the medical service given
- review the patient's medical record to ensure that his care plan is documented and followed accurately.
- documenting the day in the Case Management’s application
- Identify the vital patients with panic lab results, vital stability, long length of stay, change in the level of care, readmission, blood transfusions (cause, action, and reaction), and an inadequate number of services to the level of medical care (ward/ICU) and VIP patients
- Data collection and analysis to reach data-driven care decisions in different aspects
- Issuing case management’s daily report sent to the medical director
- Occupancy rates: bed turnover
Bachelor’s degree in one of the medical specialties (medicine, pharmacy, dentistry, or physiotherapy).(0-2) years of experience in case management profession.Comprehensive awareness of Clinical Practice Guidelines (CPGs).Preferably a Diploma in Hospital Management, Total Quality Management or MBA.Giza residency is preferred
