Job Summary
The Chronic Care Officer is responsible to raise the awareness of chronic patients by developing care plans and follow up with the patients. In addition to that, He has to analyze the utilization reports for cost containment recommendations and development of wellness tips and healthcare programs accordingly.
Duties & Responsibilities
- Audit the chronic cases and archiving medical files.
- Assure that the medications are posted and renewed regularly on the insurance system.
- Debate in the rejected chronic cases if there is a medical explanation.
- Medical follow up and monitoring the medical files of chronic patients.
- Communicate and follow up with the chronic patients.
- Coordinate clinical nutrition sessions with the chronic patients.
- Develop care plans.
- Work on generic wellness tips.
- Analysis of the accounts’ utilization reports on monthly basis.
- Develop analytical insights from the utilization reports and create consolidated dashboards.
- Develop recommendations and actions according to the consumption of each account.
- Link healthcare programs with the utilization.
